Register With The Practice?

We accept patients from Trowbridge and the surrounding villages. If you wish to join the practice please bring your medical card, completed and signed, to the main reception. If you have lost your card, alternative forms are available from reception. In this practice you may choose to consult any of the doctors and not just the doctor with whom you are registered.

Temporary Residents

You can register as a temporary resident providing you are registered with another practice in the UK. If you are temporarily living in the area, you can apply to register as a temporary resident. You will then be entitled to our full range of medical services.

New Patient Checks

It is a requirement to have an initial health check within six months of registering with the practice. This is a free health check for you and an opportunity for us to obtain important background medical history prior to your medical notes arriving. Please book your medical and bring along a specimen of urine (for checking), containers for which are available from reception.

How Do I....
Change My Details?

If you change your name, address or telephone number, please notify the surgery immediately as we need to keep our records accurate. This is very important, as a change of address may mean that you will be outside our practice area and you may be asked to transfer to the list of a doctor nearer your home. The reception staff will advise you when you notify us. Finally, if you are waiting for any hospital appointments, please ensure that you advise the hospital also of any change of address or telephone number.
