How Do I....
Obtain A Repeat Prescription?
Many patients are on long-term medication for which they have repeat prescriptions. We are pleased to provide this service for you.
It is important that requests for repeat prescriptions are made in writing preferably by using the computer slip provided. This can be presented at the reception desk or posted to us. If you do not hold a repeat prescription slip for your medication you may be advised to make an appointment with a clinical member of staff.
We ask you not to telephone for repeat prescriptions.
This is for two reasons: firstly it is easy to make mistakes with telephone requests for medicines which could be very dangerous to you. Secondly, if you occupy the telephone line for repeat prescriptions, this prevents other people telephoning in to us in emergencies.
NB: Some patients and some diseases are exempt from prescription charges. If you are in any doubt, please check with your doctor or the chemist.
Please allow 48 hours from the time your prescription is received at THE PRACTICE, to be available for collection from THE PRACTICE.
THIS FORM BELOW IS CURRENTLY DISABLED - PLEASE USE ONE OF THE ALTERNATIVE METHODS MENTIONED ABOVE TO REQUEST PRESCRIPTIONS.
